C08 covers primary malignant neoplasms of major salivary glands other than the parotid gland, principally the submandibular and sublingual glands, as well as an unspecified major salivary gland. Malignant neoplasms of salivary ducts are included.
Common Histologies
Salivary gland cancers are histologically diverse. Relevant types include:
- Adenoid cystic carcinoma
- Mucoepidermoid carcinoma
- Salivary duct carcinoma
- Acinic cell carcinoma
- Adenocarcinoma, including carcinoma ex pleomorphic adenoma
- Secretory carcinoma
- Epithelial-myoepithelial and myoepithelial carcinomas
- Squamous cell carcinoma, after excluding metastasis or direct extension from another primary site
Histologic type and grade strongly influence prognosis and treatment.
Common Symptoms
Typical presentations include:
- Persistent, enlarging submandibular (R22.1), upper-neck (R22.1), or floor-of-mouth mass (K13.70)
- Pain, tenderness, or fixation to surrounding tissues (R52)
- Dysphagia (R13.10), odynophagia (R07.0), or impaired tongue movement (G52.3)
- Numbness or other cranial nerve dysfunction (G52.9)
- Cervical lymphadenopathy (R59.0)
Pain, rapid growth, fixation, skin or mucosal involvement, and nerve deficits raise concern for aggressive or locally advanced disease. Adenoid cystic carcinoma has a particular tendency toward perineural spread.
Diagnosis
Evaluation generally includes careful examination of the oral cavity, floor of mouth, salivary glands, cranial nerves, and cervical lymph nodes. Ultrasound, contrast-enhanced CT, or MRI may define the primary lesion; MRI is especially useful when perineural spread is suspected.
Tissue diagnosis is usually obtained by fine-needle aspiration or core biopsy, followed by definitive histopathologic classification and grading. Imaging of the chest and other sites may be appropriate for advanced or high-risk disease because some tumors, particularly adenoid cystic carcinoma, can metastasize hematogenously to the lungs.
Staging
Staging considers:
- Primary tumor size and extension beyond the gland
- Involvement of adjacent soft tissue, skin, mandible, tongue, or nerves
- Regional cervical lymph-node disease
- Distant metastasis
TNM staging should use the applicable major salivary gland criteria and current staging manual. Histologic grade, margin status, perineural invasion, lymphovascular invasion, and extranodal extension provide additional prognostic information but are not encoded by C08 alone.
Molecular Markers
Selected tumors may have characteristic or actionable findings:
- Salivary duct carcinoma: androgen receptor expression; HER2 amplification or overexpression in a subset
- Adenoid cystic carcinoma: MYB or MYBL1 pathway alterations, often involving an MYB-NFIB fusion
- Mucoepidermoid carcinoma: CRTC1-MAML2 or related MAML2 rearrangements
- Secretory carcinoma: commonly ETV6-NTRK3 fusion
- Other advanced salivary carcinomas may harbor NTRK, RET, BRAF, or other potentially targetable alterations
Testing is guided by histology, disease extent, and treatment setting.
Common Treatments
Surgery is the principal treatment for resectable disease and may include excision of the affected gland with appropriate margins. Neck dissection may be performed for clinically involved nodes or when occult nodal risk is substantial.
Postoperative radiation therapy is commonly considered for high-grade tumors, advanced local disease, positive or close margins, perineural or lymphovascular invasion, nodal involvement, or other adverse features. Unresectable, recurrent, or metastatic disease may be treated with radiation, systemic therapy, biomarker-directed therapy, or clinical trials. Systemic options depend on histology and molecular findings and may include HER2-directed, androgen receptor–directed, or NTRK-targeted treatment when appropriate.

C08 identifies a primary malignant neoplasm of the submandibular gland, sublingual gland, or another/unspecified major salivary gland. It does not encode histologic subtype, grade, molecular profile, laterality, TNM stage, margin status, or treatment status. Parotid primary cancers are classified under C07. Malignancies arising in specified minor salivary glands are classified by their anatomical site, and minor salivary gland malignancy not otherwise specified is outside C08. Metastasis to a salivary gland or regional lymph node should not be represented as a primary C08 malignancy unless a primary tumor of that major salivary gland is established.
