C30 encompasses primary malignant neoplasms arising in the nasal cavity or middle-ear region. These rare, anatomically distinct cancers may present late because early symptoms can resemble inflammatory or infectious disease.
Common Histologies
- Squamous cell carcinoma, particularly in the middle ear and nasal cavity
- Intestinal-type and non-intestinal-type adenocarcinoma
- Adenoid cystic and other salivary-type carcinomas
- Sinonasal undifferentiated carcinoma
- Mucosal melanoma
- Olfactory neuroblastoma arising in the superior nasal cavity
*Superior nasal cavity primaries are captured in C30; primaries of the olfactory bulb or olfactory nerve index outside C30.*
- SMARCB1- or SMARCA4-deficient sinonasal carcinoma
- NUT carcinoma
Histologic distribution, clinical behavior, and treatment sensitivity differ substantially between nasal cavity and middle-ear tumors.
Common Symptoms
Nasal cavity tumors:
- Persistent unilateral nasal obstruction (R09.81)
- Recurrent epistaxis or blood-stained discharge (R04.0)
- Facial pain (G50.1), pressure (R51.9), or numbness (R20.0)
- Reduced sense of smell (R43.0)
- Diplopia (H53.2), proptosis (H05.20), visual change (H53.9), or cranial neuropathy with advanced disease (G53)
Middle-ear tumors:
- Persistent or bloody otorrhea (H92.10)
- Hearing loss (H91.90), ear pain (H92.09), or aural fullness (H93.299)
- Recurrent symptoms resembling chronic otitis media (H66.10)
- Facial weakness or other cranial neuropathies (R29.810)
- Vertigo or imbalance with local extension (R42)
Diagnosis
Evaluation generally includes nasal endoscopy or microscopic otologic examination followed by tissue biopsy. Contrast-enhanced CT helps assess bone erosion and detailed sinonasal or temporal-bone anatomy. MRI better defines soft-tissue, orbital, intracranial, dural, and perineural extension. PET/CT or other systemic imaging may be used to assess regional nodes and distant disease.
Expert pathologic review is often important because these sites contain multiple uncommon entities with different prognoses and treatment approaches.
Staging
Staging is site- and histology-dependent. Nasal cavity carcinomas are generally staged using applicable head-and-neck TNM criteria, including invasion of adjacent sinuses, orbit, skull base, cranial cavity, or other structures. Middle-ear and temporal-bone cancers may also be described with specialized systems, such as Pittsburgh-type temporal-bone staging; these are non-ICD constructs and are not encoded by C30. Regional cervical and parotid-area nodes and distant metastases are assessed separately.
Molecular Markers
Testing is guided by morphology and the differential diagnosis rather than performed uniformly. Relevant findings may include:
- IDH2 alterations in a subset of sinonasal undifferentiated carcinomas
- Loss of SMARCB1/INI1 or SMARCA4/BRG1 expression in deficient carcinomas
- NUTM1 rearrangement in NUT carcinoma
- High-risk HPV-associated changes in selected sinonasal carcinomas
- Histology-specific testing for melanocytic, salivary-type, or other rare tumors
Common Treatments
Management is multidisciplinary and depends on the primary site, histology, resectability, and extent. Options include:
- Endoscopic or open surgical resection for localized nasal cavity tumors
- Craniofacial or skull-base approaches when adjacent structures are involved
- Temporal-bone or middle-ear resection for selected middle-ear cancers
- Postoperative radiation for adverse pathologic features or locally advanced disease
- Definitive radiation or chemoradiation when surgery is not feasible or would cause unacceptable morbidity
- Histology- and biomarker-directed systemic therapy for unresectable, recurrent, or metastatic disease
Scope Note
C30 identifies the primary anatomic site as the nasal cavity or middle-ear region. C30.0 includes the internal nose, nasal septum, nasal vestibule, nasal concha, and nasal cartilage. Neighboring primary sites are classified separately: paranasal sinuses under C31.-, nasopharynx or posterior choana under C11.-, nasal skin under C44.3-, nasal bone under C41.0, and olfactory bulb or olfactory nerve under C72.2.
C30.1 includes the middle ear, inner ear, mastoid air cells, and eustachian tube. Primary malignancies of the external auditory canal or auricular skin are classified as skin malignancies, including C44.2- for nonmelanoma skin cancer and C43.2- for melanoma.
C30 does not encode histology, grade, TNM or other stage, molecular profile, laterality, treatment status, or recurrence. It describes a primary site and should not be used for metastasis to these structures; secondary malignant neoplasms are coded separately.
