Eligibility acknowledgement

Definition

This attachment type is a scanned or other representation of the information that the Provider may cite to prove that s/he had first checked that the patient was eligible for coverage of services to be provided, or can otherwise presume ("presumptive eligibility") that the patient is covered for such services. Sometimes such eligibility is applied retroactively after services were provided ("retroactive eligibility"). For example, a scanned image copy of the piece of paper from the County Eligibility Office for Medicaid coverage, or a form containing questions/answers to determine whether a patient is presumed eligible for pregnancy services under Medicaid or other insurance plans.
