Consent

Definition

  • Document of a patient consent for an unspecified healthcare service/procedure/activity. A patient consent form is a document that a patient signs to indicate that they give consent for (agree to allow) a particular action. Some common consent forms are for a procedure, surgery, organ donation, release of information. The actions for which a patient might be expected to give formal written consent can vary between providers, departments, facilities or regions and as with general legal documents, are numerous - thus the need for this generic term.
