Definition

  • This protocol obtains information about the personal history of respiratory symptoms and illnesses. Duration of disease, other allergic diseases, occupational history, smoking status, and family history of selected respiratory diseases are also assessed. Supplemental questions were added from other studies in areas judged to be inadequately covered by this questionnaire.
