Definition

  • This certificate is used by probate and family courts in the process of determining whether to appoint a guardian and/or conservator to assume responsibility for an individual in some or all areas of decision-making and functioning. The certificate may be completed by a physician, psychologist, certified psychiatric nurse or nurse practitioner. It contains the patient's clinical diagnosis, mental and physical condition, prognosis for improvement, and ability to make or communicate decisions about personal health and finances. The certificate also contains recommendations for level of care and supervision and appropriate treatment and/or rehabilitation.
