Definition

  • The Questionnaire form definition contains groups of questions that a clinician would like a patient to respond to. These groups of questions are not necessarily pre-defined as validated instruments but rather are selected by the clinician to create dynamic, patient-specific questionnaires. This term was created for, but not limited in use to, the HL7 CDA Questionnaire Form Definition document.
