Definition

  • The patient's risk level (very high, high, moderate, etc.) of developing a pressure ulcer based on their total score [LOINC: 38227-5] from the Braden Scale assessment. The total score can range from 6 to 23 with a lower score indicating a higher risk.[PMID: 3299278] The level of risk is used to guide the intervention strategies that should be used.
