Grad state Provider

Definition

The state in the U.S. in which the training program the provider graduated from is located. If the training program was outside of the U.S., details can be provided using the "Other, please specify" answer. This term was created for, but is not limited in use to, the American Physical Therapy Association's Physical Therapy Outcomes Registry, which defines the graduation state as the state in which the physical therapy provider graduated from an entry-level physical therapy degree program.
