- The NIH Toolbox Dimensional Change Card Sort Test raw score is the total number of accurate responses, with a ceiling of 40 points. Individuals 8 years old and older are not administered the first 10 items and instead are automatically awarded 10 points. There is no utility in administering the first 10 items to those 8 years old and older because almost all will score a 10 out of 10.
