Definition
- The risk level is calculated as the four question response total (out of a possible 400) [LOINC: 85446-3] divided by 400, then multiplied by 100 to express the risk as a percent. The ePCAM guidance recommends "Routine Care" for a risk level of 0-25% , "Active Monitoring" for 26-50%, "Plan Action" for 51-75%, and "Act Now" for 76-100%.
