Definition

  • The challenges for treatment plan maintenance panel is used to document the challenges faced by an individual patient that interfere with his or her adherence to a recommended treatment plan or health behavior. The panel contains two codes, one for recording the challenges, and the second for recording the specific areas of treatment that are affected by those challenges. This panel and the questions and answers it contains was created by a National Kidney Disease Education Program (NKDEP) expert panel.
