Encounter certificate

Definition

  • A proof of encounter certificate (or admission certificate, proof of visit document) is a document that certifies the patient was seen by a provider for a specified type of care. The certificate may be requested by the patient and submitted their insurance company for reimbursement. The document may contain patient demographics, dates admitted or seen by the care provider, and the type of intervention or care provided. It is not a summary of the visit, but rather just a document that essentially certifies that the patient was seen.
