Definition

  • The Accountable Health Communities (AHC) Health-Related Social Needs (HRSN) Screening Tool is designed to assess the the health-related social needs of Medicare and Medicaid beneficiaries, in an effort to determine impact on health care costs and health outcomes. This panel represents the supplemental questions, addressing eight additional domains: financial strain, employment, family and community support, education, physical activity, substance use, mental health, and disabilities.
